Spoilers ahead for Silent Hill 2
1st Encounter: Wood Side Apartments 2F
When venturing through the Wood Side Apartments, James will eventually find himself in the 2F section thanks to the 2F Hallway Key, and while most of the doors may be locked, there is an ominous red glow emitting from the steel bars that blocks an exit. Behind the bars is the first sighting of Pyramid Head, where players can stare him down while the monster stands with unmoving devotion to James’ torment. After this encounter, players will encounter Pyramid Head in a later cutscene that activates after entering Room 201 after theCoin Cabinet Puzzle.
Now that players are in the Blue Creek Apartments, they will understand the deadly consequences of the Otherworld. Environments caked in rot and rust plague this area, and the claw marks on the walls and floors indicate that Pyramid Head is unleashed within. Luckily, players won’t encounter the monster head-on until they solve theBlue Creep Apartments Clock Puzzleand head down to 1F through the double doors with the letter S scratched into it.
The boss fight itself is fear-inducing due to Pyramid Head’s slow walk and huge dragging sword. The boss fight itself requires players to cause damage to Pyramid Head, either with their Wooden Plank or their Handgun. Damaging him in the body or the helmet enough will cause the ground to shake, which angers Pyramid Head to make his attacks faster. The best tactic is to keep distance between James and Pyramid Head and only shoot when the gap is long enough. After enough time, a bell will ring, and Pyramid Head will leave through the main doors.
3rd Encounter: Brookhaven Hospital (Otherworld)
After solving theDirector’s Office Puzzleand heading up to the Rooftop, players will find a small room with the Page From A Diary memo on the floor. After picking this up and heading out of the doors, Pyramid Head will grab James by the neck and throw him through the floorboards back to 3F in the Otherworld version of Brookhaven Hospital.
Pyramid Head’s torment isn’t over yet, as once players solve theChained Box Puzzleand the Lady of the Door Puzzle in 2F, they will be able to venture down a staircase that will take them to a red-lit corridor where Pyramid Head takes chase. Pyramid Head will chase down James and Maria, and all players have to do is run and survive, refusing to turn back even for a moment and find sanctuary within the elevator at the end of the winding corridor.
4th Encounter: Flooded Level (Labyrinth)
Using the Rotating Cube in the Labyrinth section, players will eventually find themselves in the dimmed blue light of the Desolate Area. Navigating through this area will take players to the Flooded Level, where inside the bottom left of the map, Pyramid Head will chase James. James needs to enter His room (Pyramid Head) to both trigger theInner Sanctum trophyand destroy the breakable wall to progress. Pyramid Head can only roam the Flooded Level, so try and be quick with the wirecutters to get out fast.
5th Encounter: Ruined Area (Labyrinth)
Progress deeper into the Labyrinth with the Rotating Cube until it’s time to enter the red door on the right and progress through the Ruined Area. Each room in the Ruined Area has a red door needed to progress, but to get through it, players must first endure a fight with many monsters. Once players reach the room with a save point, the next hallway will have Pyramid Head smashing his way through a wall. This is another chase, so just run in the opposite direction to him through the Strange Wall.
6th Encounter: Two Pyramid Heads Boss Fight
Progressing through the Lakeview Hotel, players will eventually find themselves in the Otherworld, and after a cutscene with Angela and venturing through the Employee Section, players will end up in a room with 9 save options on a wall. Through the big double doors past this room will be the final Pyramid Head boss fight, where there will instead be two of them.
These Pyramid Heads both use spears, which will occasionally be thrown at James. The two Pyramid Heads aren’t too fast, so try tocreate distance and shoot them with the Rifle, or get closer to pull off a few fierce Shotgun hits and Lead Pipe swings. When enough damage has been dealt, the two Pyramid Heads will kill themselves with their spears, and they will both be holding an egg each that can be used to open the next set of doors.
